UPLIFT U OUTREACH & PREVENTION  SERVICES

Monday-Friday 9:00 am 5:00 pm (except holidays)

  The DIRECT services  we are able to provide are:
  1. FOOD assistance on a limited basis  canned goods & dry groceries
  2. CLOTHING voucher once a month  for our Metro Market Thrift  Store at 2101 N. Florida Ave.
  3. DISASTER RELIEF* if household items have been destroyed by fire, flood, etc. ­ *REQUIRES AMERICAN RED CROSS REFERRAL
  4. COMMUNITY RESOURCE INFORMATION for  other services needed
ITEMS LISTED BELOW ARE  REQUIRED EACH VISIT:
  1. PHOTO I.D. FOR APPLYING ADULT
  2. SOCIAL SECURITY CARDS/COPIES FOR ALL RELATED HOUSEHOLD MEMBERS
  3. CURRENT PROOF OF ADDRESS FOR FOOD Any Bill (electric, water, gas, phone, cable, etc.), or Rent Receipt or Lease
  4. REFERRAL FOR FOOD         
From  any agency, organization or ministry to verify needNO food referral needed for:
  1. Adults receiving SSD or SSI
  2. Senior Citizens (62+)

The Family  Care Center

(Best to come  in the morning to meet with intake counselor) BRING Photo I.D. for applying adult and Social Security cards/copies  for related  family members (spouse/children).  This is a LONG TERM, SELF-SUFFICIENCY residence program for 1) SINGLE WOMEN  or 2) FAMILIES WITH CHILDREN

WE DO NOT HAVE FINANCIAL ASSISTANCE  OR BUS PASSES
